Obtaining the Form to Request Access to the Azure OpenAI Service
Navigate to Microsoft Azure Portal - https://portal.azure.com/
Click on "More Services" within "Azure services".At the Azure services page, click More Services
Within "Categories" (on Under Categories in the left side )vertical menu, select "the AI + machine learning".
Select "Azure OpenAI" in the “Azure AI Services”-list
In the top left, click the "Create" button.
You will now be presented a window where you first have to select a Subscription and a link to the

Click the link (2) which takes you to a new page to request access to “Azure OpenAI service”.
Note: You will have to do this request to Microsoft again if you want to change subscription in the future

Submitting the form
Most of the questions and forms are self-explained, however we will focus on a few topics that might be confusing.

Under Azure AI Services + APIs, select Azure OpenAI
Click the Create button in the horizontal menu at the top. This will take on to the Create Azure OpenAI page shown below.
Select a Subscription (1) and then click the link to request access to Azure OpenAI service (2). The latter will take one to the page shown in part next below.
Filling out and Submitting the OpenAI Service Access Request Form
Most of the questions in the form are self-explanatory, and only the ones with a red star at the end of the field title needs to be filled out. However, a few of these mandatory fields might be a bit confusing and are covered here below.

Again, ensure the ID you paste here is the subscription ID of the subscription you want to use for OpenAI and not your TenantId.
Note: If you want to change the Subscription ID for OpenAI at a later stage you need to send a new application

Follow the instructions in the form and ensure that one enters the subscription ID and not the Tenant ID of the subscription. Also, note that the application is for this particular subscription ID. A new or changed subscription ID requires a new application.

Select the use-cases which fits your organizations. However, here is a suggestion for the use-cases based on our typical customer:

When done, click “Submit”. You will receive an email shortly after confirming you have submitted a request for Azure OpenAI service
Note 1: The request may take approximately 10 days but from several cases it only takes 1-2 days to get the approval.
Note 2:Submit.
Shortly thereafter one will first receive a confirmation email to later be followed by another email with the approval. In our experience, the approval email may take anything from 1 to 10 days.
