Obtaining the Form to Request Access to the Azure OpenAI Service
Navigate to Microsoft Azure Portal - https://portal.azure.com/
At the Azure services page, click More Services
Under Categories in the left side vertical menu, select the AI + machine learning option
Under Azure AI Services + APIs, select Azure OpenAI
Click the Create button in the horizontal menu at the top. This will take on to the Create Azure OpenAI page shown below.
Select a Subscription (1) and then click the link to request access to Azure OpenAI service (2). The latter will take one to the page shown in part next below.

Most of the questions in the form are self-explanatory, and only the ones with a red star at the end of the field title needs to be filled out. However, a few of these mandatory fields might be a bit confusing and are covered here below.

Shortly thereafter one will first receive a confirmation email to be followed a few days later (later be followed by another email with the approval. In our experience, the approval email may take anything from 1 to 10 days) by another email with the approval.
