Prerequisites
The user performing the steps described in this guide needs to be an 365 Admin
An Active Azure Subscription with an active price plan (tier) is required
Requesting Access to the Azure OpenAI Service
Navigate to Microsoft Azure Portal - https://portal.azure.com/
At the Azure services page, click More Services
Under Categories in the left side vertical menu, select the AI + machine learning option
Under Azure AI Services + APIs, select Azure OpenAI
Click the Create button in the horizontal menu at the top. This will take on to the Create Azure OpenAI page shown below.
Select a Subscription (1) and then click the link to request access to Azure OpenAI service (2). The latter will take one to the page shown in part next below.

Most of the questions in the form are self-explanatory, and only the ones with a red star at the end of the field title needs to be filled out. However, a few of these mandatory fields might be a bit confusing and are covered here below.
3. How many Azure Subscription ID's would you like to have access to this service?
Select 1
4. Please provide your 1st Azure Subscription ID (not your Tenant ID)
Follow the instructions in the form and ensure that one enters the subscription ID and not the Tenant ID of the subscription. Also, note that the application is for this particular subscription ID. A new or changed subscription ID requires a new application.
14. Is your organization an ISV, SI, or MSP?
If one’s company cannot be categorized by any of the given options, then use the field Other to categorize the company. Example categories: Legal Services Provider, Municipality, Consumer Goods Company, etc.
18. Which Azure OpenAI service feature(s) are you requesting access for?
Only select the Text and code models option
When done, click Submit.
Shortly thereafter one will first receive a confirmation email to be followed a few days later (we have experienced anything from 1 to 10 days) by another email with the approval.
