Personalities (aka Instructions) improvements part 3

• Removed "Instructions" from "Settings" and open it as a pop-up.

• Removed instruction toolbar

• Replaced the side panel instruction list with the new list component.

• Adjusted create-edit instruction and context form to dialog container
